Classes 2020-2024 – Please complete the form above to request your transcript. A copy of your government issued ID is required. There is a $5/transcript fee that must be paid before the transcript can be released. The fee can be paid in cash or online (see link in Google Form above).  Information on how to complete those steps is included on the form.
