Transition Services

The Student Services Transition Coordinator assists with meeting the needs of Crossnore Communities for Children’s youth with transitional services, in partnership with Crossnore’s Education Support Supervisor. The Transition Coordinator also assists all other Forsyth County group homes with transitional services that serve WS/FCS students. This role assists with educational advocacy and manages the transitional plan, which includes pre/post reporting for Crossnore’s at-risk youth. The Transition Coordinator works with various school staff, case managers, central office staff, and the child welfare system to ensure that the child’s educational needs are being met. In addition, the Transition Coordinator uses a wide range of district and local resources to provide transitional support to the area group homes.

The key responsibilities of the Student Services Transition Coordinator are:

  • Collaborate with the Crossnore Education Support Supervisor and Case Managers to develop educational plans for each youth in care.

  • Works closely with the Crossnore Education Support Supervisor to provide educational advocacy and support to family caseworkers and case managers in Crossnore’s child welfare programs; including, but not limited to, supporting networking with school systems, advocating of educational accommodations, supporting educational transitions at admission, during enrollment, and after discharge.

  • Develops and manages plans for students in transitions in area group homes that are licensed by the DHHS.

  • Attend child and family team meetings, EC/504 meetings, school meetings, and conferences related to children living in group homes, per the group home’s request.

  • Coordinate necessary academic and non-academic interventions for each youth working in close collaboration with all available resources.

  • Maintain data on youth interventions and progress.

  • Participate in BID meetings and educational status meetings with the WS/FCS Foster Care Point of Contact.
