Section 504
To be eligible for a 504 Plan, a student must have an identified diagnosis (either a mental or physical impairment) that substantially limits one or more life activities that takes place at school in comparison with the average student in the general population.
There is a broad spectrum of diagnosis that qualifies a student for a 504 Plan.
A 504 Plan is a documented plan of classroom accommodations. 504 Plans are not Individualized Education Plans (IEP). 504 Plans do not have goals. They are teacher led and directed accommodations.
A student can be considered for a 504 Plan; however, it is never a guarantee that a student will qualify for a 504 Plan. 504 eligibility is a team decision comprised of teacher(s), parent(s)/guardian(s), and the 504 coordinator. Physicians can make recommendations for a 504 plan; however, physicians do not determine 504 eligibility.
Determining 504 eligibility is a process that can take 30-60 days to complete. The steps involved include the following:
Consent to Evaluate to parent/guardian.
Classroom Evaluation - approximately 4-8 weeks of classroom observation by teachers to receive good, solid feedback. Teachers will document information during this time.
Invitation to parent/guardian for initial 504 meeting to determine eligibility.
If the team determines eligibility, then the team will move forward with writing the plan.
*Accommodations for state testing must be used regularly in the classroom and at least 30 days prior to state testing.
504 Plans are reviewed annually (eligibility is reviewed every 3 years).
504 Plans last until the 504 team determines the impairment no longer limits a major life activity OR the parent/guardian revokes the consent for services.
