Athletic Screenings are provided by Baptist Medical Center and Forsyth Medical Center and are free of charge. We are asking for a suggested donation of $10.00 in order to pay for training for Student Trainers from each of the high schools over the summer. If you are donating by check please make them payable to the Wellness Clinic.
The medical history section on the physical form must be filled out and have a parent’s signature before receiving a physical examination
