IST Forms for Teachers
 Below you will find copies of the necessary paperwork for the IST process.  Please see Ms. Cannon if you have questions about what you need to use.  I have a variety of resources available for intervention ideas and progress monitoring.  These forms are for you to print and use with parents.
 
 
Parent Notification of Screenings
This letter notifies parents that you have began the IST process for thier child.  This can be sent home with the child or given to them at your first meeting.  Complete the top portion prior to giving to parents.
 
Parent and Teacher Guide
This handout descibes the IST process.  It is a handout for the parents to take home as a reference.
 
Parent Consent for Individual Screenings
This consent form must be signed and returned by the parent before any observations, screenings, etc., can be done with the student.  Please complete the top portion and teacher signature prior to sending to parents.  This can be sent home with the student and returned to school or signed at any of the parent meetings. 
 
This paperwork must be completed for students receiving interventions.  Teacher/Parent conference form must be complete before a student can be moved to the Collaborative Conference.  Teacher/Parent Conference requires 4 weeks of interventions and progress monitoring.  Meetings include teacher and parent.
 
Collaborative Conference
This paperwork must be completed for students who have been moved from Teacher/Parent Conference to Collaborative Conference.  All progress monitoring, including paperwork and monitoring from Teacher/Parent Conference, must be complete before moving a student to IST.  Colaborative Conference requires 4 weeks of interventions and progress monitoring.  Meetings include teacher, parent and another professional.
 
Please make sure that all forms, signatures and progress monitoring are complete prior to referring a student to the Intervention Support Team.  When you are ready to refer, bring the completed paperwork to Ms. Cannon.
